Here's a blank permission slip in MS Word format that you can fill out and print for use at Troop 403 events.  When the event will involve adults drving scouts then the second page must also be filled in and printed. 

As a general rule you need 1/3 as many copies of the adult permission slips as that of the scout permission slips.

You should deliver the copies of the slips to the troop treasurer the troop meeting night you announce the event.
